Job Summary :
• Provides comprehensive case management and care coordination for a diverse panel of patients with identified needs spanning age, condition, and population variables.
• Coaches patients to improve their health behavior and attain health-related goals while working as part of a healthcare team.
Key Responsibilities :
• Collaborate with providers, nurses, social workers, patients, families, and caregivers to ensure comprehensive care.
• Recognize and reward performance in a challenging environment.
• Provide clear direction for success in the role and development for other interested positions.
Required Qualifications :
• Graduate from an accredited school of nursing.
• Active, unrestricted licensure as a Registered Nurse in a state.
• Current state driver’s license with the ability to travel to hospitals, skilled nursing facilities, patients' homes, and other care sites as needed.
• Full COVID-19 vaccination is an essential job function; compliance with state booster requirements is necessary.
• Ability to perform all essential job functions with or without reasonable accommodation.
Preferred Qualifications :
• Certification in Geriatric Nursing or Geriatric Case Management.
• 5+ years of nursing experience, preferably with older patients.
• Proficient in computer use, the Internet, and health information technology.
• Demonstrated excellent communication, interpersonal, and organizational skills with a flexible approach to problem-solving.
• Proven ability to work effectively both independently and as a member of an interdisciplinary team.
