Summary

Please Note : This is a pool, as needed position. No guarantee of hours. The speech language pathologist provides home care services in community based and facility settings.

The position is responsible for providing therapy services to patients enrolled in home health and hospice programs under the direction of the patient’s physician.

The speech language pathologist’s services include diagnosing disorders and providing therapeutic interventions to patients who demonstrate disorders of communication and / or swallowing as referred by a Licensed Health Care Professional (LHCP).

Quality of care is provided in accordance with state, federal and accreditation body regulations and the mission of Home Care Services.

Position Reports To : Rehab Supervisor and agency leadership

Essential Functions

  • Performs a comprehensive speech and language patient assessment, evaluates the patient's rehab potential, home safety, equipment needs, psychosocial status, need for community resources and home health / hospice services through skilled observation and administration of age-specific tests.
  • Interprets evaluation and test results and develops a plan of care with patient specific therapy goals to help patients develop, regain or maintain optimum function and safety in their home environment and community.
  • Administers skilled treatment interventions as prescribed by the physician that may include but are not limited to utilization of therapeutic exercises / activities, modalities, recommendation of adaptive equipment, compensatory techniques and instructing the patient / family in home programs to be continued in the therapist’s absence, when indicated.
  • Reassesses the status of the patient’s functional ability and the effectiveness of the speech therapy plan of care and makes appropriate revisions with the physician’s approval.
  • Coordinates the delivery of speech therapy services with nursing, social work, aide, therapy, equipment suppliers, community resources and transitions of care which results in effective utilization of resources and timely attainment of achievable rehabilitation and / or palliative care goals.
  • Participates in weekly interdisciplinary team meetings and agency meetings as assigned.
  • Records, and reports to physician, the patient’s reaction to treatment, any changes in the patient’s condition, suggested changes in the patient’s orders and implementation of discharge planning procedures.
  • Maintains documentation of evaluation, reassessment, subsequent visits, coordination of care, discharge or transfer summaries and other clinical record documentation in the electronic medical record.
  • Instructs and supervises other health team members in speech therapy procedures and techniques performed as an adjunct to the regular speech therapy plan of care.
  • Required to maintain active Basic Life Support (BLS) certification.
  • Other duties as assigned

Qualifications

Required :

  • Masters’ Degree in Speech Language Pathology
  • Current WA State Speech Language Pathology license.
  • A certification of clinical competence by the American Speech and Hearing Association
  • Communication : Functionally able to listen, read, speak and write using English language. Functional hearing and visual acuity sufficient to detect changes in patient conditions and perform assessments.

Effective interpersonal skill. Demonstrates ability to work in collaboration with health team member.

  • Must possess basic computer / electronic device skills related to Windows and app navigation, mouse and screen usage, keyboarding, email communication and password management.
  • Current, valid driver's license and access to an insured vehicle with the minimum coverage by law.

Desired :

  • One year of recent speech language pathology experience.
  • Previous home care, acute care or skilled nursing experience.

Work is performed primarily in the patient's home environment or a facility. May also work from the office as needed. Involves continuous contact with patients.
