Job Summary
We are looking for individual to join our client’s team as Licensed Master Social Worker. Under supervision provides Social Work services to patients attending the Outpatient Mental health / Chemical dependency service.
Engages in independent decision-making about psychosocial needs, provides psychosocial services to individuals and their families, and provides formal and informal meetings regarding patient and family communication and decision making.
Provides direct patient care as well as consultative services to assure the delivery of state-of- the-art care.
Job Description
- Interview assigned clients and completed a bio-psychosocial history with an initial intake and a comprehensive treatment plan.
- Complete Master treatment plan, reviews, and updates treatment plans quarterly with patients.
- Provide supportive and educational counselling and psychotherapy
- Facilitate psychotherapy / psychoeducational groups
- Completes referrals to social welfare and other community agencies
- Contact patients’ families, collaterals, significant others as well as well as community and medical and include share this information in assessment, documentation and during team meetings.
- Participate in multidisciplinary team meetings to coordinate patient therapeutic care.
- Documents all interventions, outreach efforts and client progress in the medical record in compliance with OMH / TJC / Hospital policies and procedures.
- Submits weekly / monthly statistics.
- Attends supervisory conferences
- Serve as Field instructor for MSW interns (Optional at supervisor / Social worker discretion)
- Provides crisis assessment and intervention in the Adult Outpatient clinic.
- Act as a consultant to other program clinicians to provide referrals for economic social and psychological services as needed.
- Complete other assigned tasks by the supervisor.
- Promote a positive and pleasant attitude conducive to the workplace, which enhances and furthers the Mission Vision and value statement.
- Meets all compliance standards / requirements such as annual physical evaluation, annual in-service fire drill participation etc.
- Participates in performance improvement activities
- Be responsible for enhancing personal and professional development by attending training sessions.

Skills Required
One-year LMSW experience
Education / Training / Certifications
- Licensed Master Social Worker
- Valid Licensed Master of Social Work (LMSW), license issued by NYSED, Primary Source Verification required.
Additional Requirements
Malpractice Insurance - All clinical workers are required to have active individual Medical Malpractice. A copy must be provided prior to start of assignment.
All clinical workers licensed by the state of NY are required to provide.
Pay Rate
