Transitions Healthcare is looking for a qualified Nurse Practitioner to join our growing health care team.

Transitions Chronic Care and Transitions Home Health Care were created in 2012 to address the fragmentation of care inherent to our healthcare system.

Our mission is to improve the patient experience while producing better health outcomes at a lower cost. It is the role of our Nurse Practitioners that allows us to accomplish our mission.

Our Nurse Practitioners work both independently and in partnership with physicians, nursing staff, and other healthcare professionals to ensure optimal care across a variety of care settings.

You will function as a chronic care / palliative care specialist with emphasis on managing and coordinating the complex chronic care and transitions of care for our patients as well as assisting the Home Health operations as needed.

The Transitions Healthcare providers and support staff are passionate about the mission of this innovative and collaborative model of care.

We understand the extra time and support it takes to care for patients with serious illness. You will have direct interaction with your patients delivered in a variety of locations that may include hospitals, skilled nursing facilities, assisted living facilities, long term care facilities and the patient’s home.
