About the Position

• Community Health Worker - Substance Use Disorder
• Part-time .8 FTE (64 hours per pay period) to last approximately 11 months. The role's functions and will be assessed prior to the eleven-month mark to determine continuation of FTE.
• On-site at Grand Itasca Clinic & Hospital in Grand Rapids, MN.

Job Summary :

The Community Health Worker (CHW) is responsible for helping community members, patients and their family's access and navigate community services and resources and adopt healthy behaviors, specifically with individuals struggling with substance use disorder (SUD). The role also supports patients and families experiencing neonatal abstinence syndrome (NAS). The CHW supports providers and team members through an integrated approach to care management and community outreach. The role's actions will promote, maintain, and improve individual and community health. The CHW provides social support and informal counseling, individual and group health education and advocacy for individuals and community health needs. Providing home visits and health screenings may be required. This position is a part of a Health Resource Services Administration (HRSA) Rural Communities Opioid Response Program (RCORP) grant received by Grand Itasca Clinic & Hospital, where funding will be used to strengthen and expand SUD / opioid use disorder (OUD) prevention, treatment, and recovery service delivery in our community.

The grant has eleven months remaining on the monetary payment term. The sustainability of this role will be assessed prior to the eleven-month mark to determine continuation.

RCORP Description :

Over the funding period, Grand Itasca's RCORP project will :

Increase access to behavioral health care, specifically for those experiencing SUD / OUD, and more explicitly, for rural pregnant and postpartum persons and their families; and
