POSITION SUMMARY : The Assistant Community Care Manager will provide support to Community Manager (s) with planning, directing and organizing the delivery of LIfeStyle Options service sin a manner that ensures quality standards.
ESSENTIAL DUTIES :
- Care coordination for residents in retirement community
- Monitor Care plans to ensure resident needs continue to be met.
- Coordinates client weekly billing and transmittal
- Conducts client evaluations
- Fill-in clients services as needed.
- Conducts caregiver orientation / training, as needed
- Coordination of client and caregiver schedules, approval of timeslips and entering corrections / changes in the scheduling software.
- Caregiver coaching and counseling as required
- Performs the duties of the Community Manager in their absence.
- Other duties may be assigned
PERFORMANCE RESPONSIBILITIES :
- Maintains positive internal and external customer service relationships
- Maintains open lines of communication
- Plans and organizes work effectively and ensures its completion
- Meets all productivity requirements
- Demonstrates team behavior and promotes a team-oriented environment
- Actively participates in continuous quality improvement
- Represents the organization professionally at all times
POSITION REQUIREMENTS & COMPETENCIES :
- Certified Nurses Aide in good standing with the state is preferred
- Two (2) years health care experience preferred.
- Ability to work independently
- Must have a valid drivers license and reliable vehicle
