A little more about our center :

Southview Manor (formerly Southview Care) is an inner-city skilled nursing facility (SNF) with 5 floors and an average census of 160+.

We have and multiple levels of care, including long term care and a large behavioral health (formerly indigent) population.

• Highly ranked Quality Measures this means we provide excellent clinical care!
• The average age of our patients is 35-55 and the most common diagnoses are schizophrenia, bi-polar and major depressive disorders.
• Many are able to age in place, and about 25% return to the community.
• We have several programs encouraging engagement, but the most popular spot in our center is the game room.
• We love new grads and work with students from multiple schools / universities in the area.
• The Metra and the Green Line are just a few blocks away

Job Description

What you will do in this role : The Psychiatric Rehabilitation Services Coordinator (PRSC) provides group and individual therapy.

This includes psychiatric rehab and case management services to adults with a history of psychiatric (Behavioral Health) hospitalizations.

You will help them with long-term care and / or stabilization. You will be a part of our interdisciplinary team in developing and implementing therapeutic services and interventions to best serve their needs.

Qualifications

Psychiatric Rehabilitation Services Coordinator (PRSC) : officially we title this job a PRSC, but in other centers it may be called a Psychosocial Rehabilitation Services Coordinator, Psychiatric Rehab Coordinator, Psychiatric Rehab Counselor, or Mental or Behavioral Health Counselor

What we need from you :

• Prior experience as a PRSC or mental health professional in a licensed nursing home, intermediate care facility, or specialized mental health rehab facility (SMHRF) is preferred
• Bachelor’s Degree in a human services field (including but not limited to sociology, special education, rehabilitation counseling or psychology) and have a minimum of 1 year of supervised experience in mental health or human services setting
• Experience with Point Click Care (PCC) is ideal, but not required
