Overview

A licensed practical nurse is a clinically competent position that provides care to a group of home health patients / residents under general supervision / delegation of a registered nurse according to established policies, procedures, protocols and standards of practice and care. Effectively interacts with patients / residents, families, responsible party, and all health team members to assure resident safety.

Responsibilities

Perform initial data collection on all patients / residents and follows up on data collection as per policy.

• Participate in implementing person driven plans of care as developed by the Registered Nurse within established policies and procedures and continually works to meet individual patient / resident needs.
• Demonstrate knowledge of medications and their correct administration based on age of the patient / resident and their clinical condition. Observe, document and report patient / resident responses to medications.
• Follow patient / resident, family and caregiver education plan of care as developed by the Registered Nurse, clearly communicates information and documents patient / resident response to education.
• Document according to current standards and policies.
• Plan for patient / resident discharge, collaborates with the Registered Nurse and discharge planners for long and short term patient / resident needs.
• Coordinate and direct patient / resident care appropriately.
• Perform charge nurse or building responsibilities when assigned.
• Utilize the principles of Relationship Based Care while performing duties.
• Promotes the Benedictine Mission and Core Values of Hospitality, Stewardship, Respect and Justice by bringing the Mission and Core Values into the day-to-day activities of the company.
• Performs other duties, tasks and / or projects as assigned.
