About The Position
Pioneer Healthcare Services is seeking a committed Home Health Physical Therapist to join our team in Portland, OR for a 13-week assignment.
The qualified HH PT will act as the patients' partner throughout their daily tasks and assist patients to function at their personal best.
You will maximize the quality of life and movement potential within the spheres of promotion, prevention, diagnosis, treatment or intervention, and rehabilitation.
Responsibilities of a Home Health Physical Therapist : One year of home health experience preferred Developing and revising the plan of care in consultation with the physician and other health care professions Evaluates and treats patients recovering from injury, disease, or surgery in a home care environment Preparing clinical progress notes, evaluating outcomes Establishes treatment plans using a variety of methods Able to respond to inquiries and complaints from employees and regulatory agencies Meet all productivity standards and requirements daily Must have clean driving record to apply 40 hours guaranteed 13 week assignment Submit all billing logs and necessary information on a timely basis New grads and experienced therapists are encouraged to apply - We will support you through this transition
