As a Home Health / Hospice Licensed Practical Nurse (LPN), you wil l :

• Present the Home Health / Hospice benefit to patients and their families / representatives
• Assess signs and symptoms of discomfort including presence of pain and takes prompt and appropriate action
• Follow established plan of care while collaborating and communicating with RN Case Manager and Interdisciplinary Team, including physician, as appropriate
• Communicate with patients’ family or friends to provide advice, comfort, and release instructions.

To qualify for a Home Health / Hospice Licensed Practical Nurse (LPN) with us, you will need :

• Licensure : Current license to practice as a Licensed Practical Nurse (LPN) in the state associated with this position
• Current CPR / AED / BLS / First Aid certification
• Reliable transportation to / from care sites and / or work locations
• Valid state-issued driver’s license, automobile insurance, and reliable transportation; willingness to travel
• One (1) year preferred experience practicing as a Licensed Practical Nurse (LPN) in a hospice or similar setting
• Hospice bedside and continuous care experience preferred
