Responsibilities Home health setting serving Medford, OR and surrounding areas Day shifts with a 5x8 hour schedule, typically 8 : 00 to 16 : 30 Patient ratio is 1 : 1, with up to 6 home visits per day and a 30 productivity point standard No weekends or on-call required No floating required between like units, but may float to other facilities within the state if needed Uses EPIC and Oasis for charting; Oasis experience required Travel may include occasional drives up to a 50 mile radius Mileage reimbursement provided at IRS standard Primary responsibilities include delivering skilled nursing care to home health patients, completing Oasis assessments, and maintaining accurate documentation in EPIC System organizes client visits to minimize drive and windshield time Former permanent employees must wait at least three months before returning as a traveler and cannot apply if within 50 miles of the facility Minimal to no RTO preferred; significant time-off requests may impact consideration Local candidates within 50-75 miles accepted at the same rate Free parking available Any color scrubs may be worn Compliance modules must be completed pre-start and annually First-time travelers are not eligible for this position Requirements Active OR RN license required prior to start At least one year of recent skilled home health RN experience required BCLS certification required Strong proficiency and recent experience with Oasis documentation required Valid driver's license and reliable personal transportation required Completion of all required health screenings, including COVID primary series and at least one booster Able to provide two supervisory references covering at least one year within the last three years in a comparable specialty Benefits High Pay and Bonuses Medical, Dental and Vision Insurance with Day 1 Options 401(k) Plan Weekly Payroll Deposit Free Online CEUs Generous Housing Allowance Travel and Licensure Reimbursements Non-taxed Per Diem and Subsidy Traveler Rewards and Discounts American Traveler Job ID P-624665.
