About the Role :

The Hospice Continuous Care Nurse plays a critical role in providing compassionate and comprehensive care to patients in their final stages of life. This position is focused on delivering high-quality nursing care that addresses both the physical and emotional needs of patients and their families. The CC Nurse will work closely with an interdisciplinary team to develop and implement individualized care plans that ensure comfort and dignity for patients. By providing continuous care, the CC Nurse helps to manage symptoms and alleviate pain, allowing patients to spend their remaining time in a supportive environment. Ultimately, the goal of this role is to enhance the quality of life for patients and provide support to their loved ones during a challenging time.

Essential Job Functions / Responsibilities

• Manage a patient’s symptoms during a short term medical crisis.
• Provides direct patient care as defined in State Nurse Practice Act.
• Recognize and interpret symptoms, institute remedial measures and report significant changes in the patient condition immediately.
• Chart the patient’s condition hourly to provide accurate and timely documentation of patient services to reflect the plan of care
• Implement current nursing practice following a comprehensive assessment and the plan of care.
• Assess and provide patient and family / caregiver education and information pertinent to diagnosis and self plan of care.
• Uses equipment and supplies effectively and efficiently.
• Provides appropriate pain / symptom management. Evaluates patient’s response to treatments / medications.
• Participates in personal, professional growth and development. Also participates in quality assessment performance improvement teams and activities.
• Performs other duties as assigned by the registered nurse.
• Should be able to travel to other areas.

Continuous Care nurses are a special part of the hospice team, as they support both patient and family in the last stages of life or crisis. Continuous Care shifts are typically 12 hours done at the patient’s bed side.

The required skills for this role include strong clinical assessment abilities, which are essential for identifying patient needs and developing appropriate care plans. Effective communication skills are crucial, as the CC Nurse must convey complex medical information to patients and families in an understandable manner. Compassion and empathy are vital in providing emotional support to patients and their loved ones during difficult times. Time management skills are necessary to prioritize tasks and ensure that all patients receive timely care. Additionally, familiarity with electronic health records (EHR) systems is beneficial for accurate documentation and coordination of care.

Pay : $30.00 - $37.00 per hour
