Key Responsibilities
Manage data and registry for special populations, assessing progress and proposing quality improvement initiatives
Coordinate care for complex members, including training staff on the Collaborative Care Model
Provide direct member care through outreach, screenings, and education on health-related issues
Required Qualifications
Registered Nurse (RN) or Licensed Practical Nurse (LPN) with SAMHSA required training for an X DEA license
Bachelor's degree in Nursing preferred
At least 5 years of experience in addiction medicine or related behavioral health fields
Understanding of care coordination, case management, and healthcare regulations
Access to reliable internet and telephone services for remote work
