Physical Therapist (Home Care) - PACE Elder Care Services - Full-Time

Summary :

The Physical Therapist role focuses on participating on an interdisciplinary team level to support and advise on therapeutic needs of program participants, providing input on participant care plans, and maintaining a caseload of participants who can be treated both in the home and in an outpatient gym setting located in our Adult Day Health Center.

We are a unique preventative care model compared to the reactive health care systems to which most home care agencies adhere.

In Accordance with CHA policies and procedures, federal and state regulations, professional standards of care, and APTA guidelines and code of ethics, the physical therapist evaluates and treats program participants in their home, as well as the PACE Center Clinic, with a focus on quality care, service excellence, cultural competence, and performance improvement.

This position is team-based home care without the utilization of OASIS typically used in home care visits.

Qualifications / Requirements

Education / Training : Bachelors, Masters, or Doctoral Degree in Physical Therapy

Licensure : Massachusetts PT License

Certifications : ARC or AHA BLS for Healthcare Providers

Work Experience : 0 years, 1+ preferred
