Community based nursing allows Mercy Home Health to provide a personal touch to the patients we meet while seeing them through to success.

Overview :

This nurse will be responsible for coordinating palliative care of patients in Home Care and advising / assisting those co-workers in identifying patients that need to be transitioned to Hospice.

This nurse may also case manage some patients from the transition period throughout their Hospice length of stay.

Qualifications :

• Experience : Two or more years related experience.
• Required Education : Graduation from accredited nursing school.
• Licensure : Current RN license in the state of Missouri.
• Preferred Certifications : Current certification as a Hospice and Palliative Nurse.
• Certifications : BLS CPR Certification through American Heart Association.
• Other : Current state driver's license and auto insurance.
