NURSE PRACTITIONER
Hospice & Palliative Care of Iredell County is recruiting for a full-time Palliative Care Nurse Practitioner to provide care to patients with advanced life threatening / life limiting illnesses in Iredell County and surrounding areas.
Candidates should be caring, self-directed, and comfortable with providing care in a variety of settings including homecare, nursing facilities and hospice inpatient units.
This position will primarily work with the Palliative Care Interdisciplinary Team to provide hospice & palliative care in an outpatient setting under the direction of the Chief Medical Officer.
Responsibilities include pain and symptom management, education and counseling for patients and families regarding disease progression and prognosis, completion of Advanced Care Directives and serving as an educational resource both within the organization and outside community.
Competitive salary and full-time benefit package also available. Two years nursing experience with home health / hospice and / or facility experience preferred.
Advanced understanding of the end-of-life care process required.
