Job Requirements :
Must Be HHA Certified
MUST SUBMIT :
- Valid original NYS HHA / PCA certificate
- Valid I-9 Information to establish both identity and employment authorization
- Two (2) Reference letters - One Personal and One professional
- Pre-employment physical - Within the last 12 months.
- Negative PPD, Chest X-Ray (if PPD positive)
- Rubella / Rubeola and Drug Screen (Lab-work attached).
