- Electronic Medical Record (EMR)
- Other EMR
- TruChart EHR
- 1 plus years of experience in ANY of the following :
- Acute Care
- Rehabilitative Setting
- Valid Physical Therapy Assistant in the state of Illinois (IL)
- Valid Regular Driver License (DL)
- Degree in Physical Therapy
Preferred :
1 plus years of experience in Home Health

Our client seeks a compassionate Home Health Physical Therapy Assistant to join their extraordinary care team in Springfield, IL. The role involves :
- Collaborating to provide patient care plans.
- Supporting patients in the area.
- Demonstrating skills in time management and technology use.
Key Responsibilities :
- Collaborate to provide management and delivery of patient care plans.
- Support patients in Springfield, IL, and surrounding areas.
- Consistently meet critical deadlines for charting
- Self-manage time and schedule.
Schedule and Shift Details :
Traditional Monday through Friday from 8 : 00 a.m. - 5 : 00 p.m.
