- Perform comprehensive assessments of psychosocial, medical, and discharge needs of patients and their families.- Coordinate the plan of care and drive the discharge plan in collaboration with the multidisciplinary team.
- Work collaboratively with RN case managers and the multidisciplinary team.- Perform assessments and develop appropriate discharge plans for high-risk patients with discharge planning or psychosocial needs.
- Engage in consultation, discharge planning, education, counseling, advocacy, and information referrals to meet the emotional, social, and financial aspects of care.
- Maintain calm during crises and deliver information clearly and concisely.- Build trust and act with authenticity while multitasking and prioritizing workflow and needs.
- Adapt to changes and work effectively as a team player.- Follow business casual dress code.
