Certification Details
- License / Certificate Verification - current license and primary source verification required 30 days prior to start of assignment
- Ascension Criminal Background Check Results required
- Ascension Education Verification must be validated at the source level
Job Requirements
- Must have BLS certification - Must be AHA.
- Must have ACLS certification - Must be AHA.
- Pharmacology 1,2 exam required
- NIH Certification required
- Ascension EKG Exam - Score needs to be 90% or higher with only 2 attempts.

Ascension Unit Specific Checklist - Optional for Onboarding, but must be uploaded by the unit leader within a week.
Additional Details
- Vaccines or Positive IGG Titer required; Exemptions are NOT accepted for certain areas.
- Documentation of Hepatitis B vaccination series completion or declination documentation required
- Influenza Vaccine required for flu season starting Sept 4th, 2023; no declinations or exemptions accepted.
- Ascension Onboarding Requirements Education Attestation must be uploaded by the Agency.
- Ascension Nursing Contract Worker Onboarding to be done onsite.
