Community Reach of Montgomery County is a nonprofit organization that has been serving Montgomery County, Maryland for over 50 years.
We provide human services to those in need through five service areas : 1. Healthcare, 2. Housing, 3. Financial Emergency Assistance, 4.

Responsibilities of the position :
Case Management (75% of time) : Qualifies and orients new clients to the Housing and Senior Reach programs. Collects, completes, and submits documentation as required to program funders.
Provides ongoing, intensive case management to 11 Housing clients who have previously experienced homelessness; provides low-impact case management to approximately 35 seniors.
Assists with finding and applying for resources, health services, and training. Communicates with family members, homecare providers, treatment providers, additional caseworkers, medical providers, and volunteers.
Facilitates monthly group meetings of housing residents, and mediates conflict between residents. Schedules and confirms homecare services for Senior Reach clients.
With Director, Property Services, organizes and promotes workshops and one-on-one learning opportunities for clients. Provides on-call coverage for Housing Program.
Administrative Duties (25% of time) : Keeps accurate and up-to-date records of client documents, services provided, communications.
Updates financial eligibility as required by funders. Uses the Montgomery County Homeless Management Information System (HMIS).
Negotiates contracts and relationships with outside vendors. Tracks expenses and assists in the creation of budgets. Records in-kind donations of time (volunteers) and goods.
Maintains program statistics, and assists in preparation of reports. Serves as representative of Housing and Senior Reach programs at various community events and meetings with partners.
This position works collaboratively with the Director, Property Services, Housing and Senior Reach.
Qualifications :
- A bachelor's degree in a human services field and two years of case management experience;
- Proficient in Microsoft Outlook, Word, Excel, and Teams within Office 365;
- Valid driver’s license and reliable transportation.
Preferred Qualifications :
- Program or project and budget management;
- Experience supporting and building trust with clients who have been unhoused, have a history of substance abuse, or with an elderly population and their families;
- Knowledge of the Montgomery County HMIS system;
- Experience working collaboratively with case workers and providers from outside organizations;
- Adaptable in real-time to match the changing demands of different situations;
- Experience writing creative service plans to identify strengths and set goals;
- Able to locate resources for clients with varying needs.
Salary Range : $52,000 to $56,000 annually
