POSITION SUMMARY
The Health Services Director oversees the health services department and ensures resident care needsare met by coordinating resident care, managing health services systems, and providing supervision tohealth services team members.
Promotes a positive physical and psychosocial environment for theresidents.
ESSENTIAL DUTIES AND RESPONSIBILITIES
1. Creates a resident-centered approach to the planning and delivery of services to residents, supportingresidents’ rights to participate in decision-making and treating residents with dignity and respect.
2. Performs assessments on health, functional and psychosocial status of residents. Participates in thedevelopment of individualized service plans.
Proactively manages care and services for each resident,maximizing the resident’s opportunity to remain in the assisted living environment.
Problem-solves aspart of the management team to case manage and meet the needs of residents.
3. Assesses or evaluates residents with reported changes of condition. Provides team member direction,service planning, and coordination of care to meet the residents changing needs.
4. Collaborates with the management team on the move in of prospective residents, taking intoconsideration the needs of the prospective resident, current community acuity, and staffing levels.
Provides in-person pre-move in assessments, if necessary.
5. Ensures that resident care needs are met by coordinating services with other professionals, includingphysicians, discharge planners, nurses, home health agency staff, case managers, hospice, and stateagency personnel.
Oversees the arrangement of ancillary services such as pharmacy, podiatry, lab, andx-ray as needed.
6. Communicates regularly with residents’ families regarding health care needs. Provides education andsupport to residents and families.
7. Provides supervision and training to health services team members including personal care andmedication management. Manages the delegation of nursing tasks as allowed under applicable statenursing laws and practice acts.
8. Partners in the investigation and root cause analysis of incidents. Collaborates with the managementteam on the development and implementation of appropriate interventions.
9. Oversees the documentation and communication of resident care and services. Ensures residentrecords are completed in a timely manner and maintained according to company policies and stateregulations.
10. Assists in the management of Infection Control. Provides training to staff on infection controlpractices, investigates, and reports incidents of infection and tracks infection data.
11. Manages and oversees health services systems. Maintains up-to-date knowledge of all rules,regulations and policies pertaining to resident care.
Stays informed of industry and health caretrends and standards, applying new knowledge to meet the changing needs of residents.
12. Provides on-call support and phone triage for team members’ questions regarding resident concerns.
13. Participates in leadership team by actively contributing in decision making, incident follow up, buildingand company-wide initiatives and attending leadership team meetings such as daily stand up, SystemsMonitoring and Resident Tracking (SMART), Quality Assurance Performance Improvement (QAPI), andother meetings as required.
14. Acts as an ambassador of Prestige by representing the community in a positive manner to outsideagencies and participating in community events and social functions.
15. Performs other duties as assigned.

QUALIFICATIONS AND EXPERIENCE
Must have a current, unencumbered LPN / LVN or RN license in the state you are practicing. RNstrongly preferred. Experience in geriatric nursing, medication administration, case management,training / staff development, budgets, and team management preferred.
EDUCATION
LPN / LVN, RN, Associate Degree in Nursing, or Bachelor of Science in Nursing.
