SUMMARY :

The RN Nurse Care Manager is responsible for delivering comprehensive care management services within an integrated primary care setting.

This includes conducting assessments, developing care plans, coordinating patient care, and collaborating closely with primary care providers to address patients' medical, psychosocial, and educational needs.

The role requires regular on-site presence to ensure seamless integration with the healthcare team.

ESSENTIAL JOB DUTIES AND RESPONSIBILITIES :

  • Supports and implements Gracelight’s vision, mission, and values; fosters and promotes a culture of service excellence and accountability.
  • Performs all job functions in a professional and courteous manner, including timely answering of phone calls and emails and providing excellent customer service to internal and external customers.
  • Delivers care management services in an integrated primary care environment, working collaboratively with medical providers to offer holistic, patient-centered care.
  • Conducts comprehensive nursing assessments and develops individualized care plans in coordination with primary care providers and other members of the care team.
  • Monitors patients' progress towards health goals by tracking adherence to treatment plans, coordinating follow-up care, and addressing any barriers to care.
  • Provides patient education on disease management and self-care strategies to enhance patient outcomes and empower patients in their healthcare journey.
  • Consults with primary care providers to enhance understanding of the patient’s needs and to assist with implementing comprehensive treatment and care plans.
  • Collaboratively develops multi-disciplinary workflows, clinical pathways, and other practices to maximize the benefits of team care for patients.
  • Coordinates care across multiple providers and settings to ensure continuity and quality of care.
  • Responds promptly to primary medical care staff for crisis intervention and supports staff during emergencies.
  • Maintains daily visit and caseload targets to ensure optimal patient care and service delivery.
  • Completes all documentation in a timely manner and ensures accuracy in patient records, care plans, and billing documentation.
  • Complies with organizational policies and procedures and adheres to HIPAA regulations.
  • Demonstrates respectful and collaborative behavior with colleagues, patients, and supervisors.
  • Supports patient involvement in planning, implementing, and evaluating care services.
  • Completes data collection forms and prepares reports as required.
  • Demonstrates flexibility to change and to evolving healthcare needs, organizational changes, and advancements in the field.
  • Must be willing and able to work at all locations as needed to meet patient care needs.
  • Must be willing and able to work all business hours, including evenings and weekends.
  • Performs other duties as assigned.

QUALIFICATIONS, SKILLS & ABILITIES :

  • Registered Nurse (RN) licensed in California.
  • Bachelor's degree in nursing (BSN) preferred.
  • Experience in care management, preferably in a primary care setting.
  • Strong clinical and care coordination skills.
  • Excellent written and verbal communication skills, utilizing tact, active listening, empathy, and compassion. Able to manage conflict in a direct and professional manner.
  • Ability to work effectively in a team-based care environment.
  • Proficiency in electronic health record systems and documentation.
  • Experience working with diverse populations from various social, cultural, educational, and economic backgrounds.
  • Valid driver’s license with current automobile insurance.
  • Current CPR certification required.
  • Ability to organize and prioritize work tasks.
  • Proficiency with computer applications such as Microsoft Excel, Word, and PowerPoint; EPIC.
  • Bilingual (Spanish, preferred).
  • Strong problem-solving and critical thinking skills.
  • High level of cultural competence and sensitivity.
  • Flexibility and adaptability in dynamic work environments.
  • Strong organizational skills and attention to detail.
  • Regular on-site presence required.

Physical : Regularly required to use hands to finger, handle, or feel and reach with hands and arms. Frequently required to stand, walk, and sit.

Occasionally required to climb or balance, stoop, kneel, crouch, or sit. Occasionally required to push / pull objects up to 50 lbs and to lift / carry objects up to 25 lbs.

Frequently required to perform moderately difficult manipulative tasks such as typing, writing, etc. Occasionally required to ascend and descend stairs.

Specific vision abilities required by the job include close vision, color vision, and ability to adjust focus.

Sensory : Ability to compose routine reports and correspondence. Must be able to distinguish normal sounds with some background noise, as in answering the phone, interacting with staff, etc.

Must be able to speak clearly and understand / be understood using the English language.

Cognitive : Frequently required to concentrate on moderate detail with constant interruption. Frequently required to understand and relate to specific ideas, several at a time.

Must be able to analyze information, problems, situations, practices, or procedures. Must be able to analyze complex technical data using qualitative and quantitative sources of information to formulate logical and objective conclusions.

Must be able to carry out instructions delivered in written, oral or in other daily situations that arise and deal with problems involving several concrete variables in standardized situations.

Environmental : Frequent exposure to varied office and mixed (health center / office) environments. Occasional exposure to toxins and poisonous substances, dust, and loud noises.

SPECIAL REQUIREMENTS :

  • Must be able to meet and receive a criminal records clearance, as required by Title XXII, other licensing regulations, and Gracelight practices.
  • This position may require occasional driving for business purposes. A valid California driver's license, reliable personal vehicle, current personal auto insurance as required by law, and an MVR (Motor Vehicle Record) sufficient to obtain and reasonably maintain insurability under the agency's auto liability policies are essential job requirements.
  • A personal cell phone with reliable service and a data plan to use for business purposes.
  • May be required to obtain and maintain First Aid and CPR certification.
