Length : 13 or 26 weeks

On-Call / Weekends : Every 3rd weekend, on-call once weekly as needed

• Patient caseload : 25-30 patients for case managers, 6-7 visits daily for non-case managers
• Additional information : New EMR HomeCare HomeBase implementation occurring, will receive training on new EMR
• COVID-19 Vaccine (Facility Guideline) : Not Required

Home Health RN

Schedule : Typically 5x8's Days 0On-Call / Weekends : Every 3rd weekend, on-call once weekly as needed

Patient caseload : 25-30 patients for case managers, 6-7 visits daily for non-case managers

Years of Experience : Minimum 1 year in home health

Length of assignment : 13 or 26 weeks available

Additional information : New EMR HomeCare HomeBase implementation occurring, will receive training on new EMR.

Required certifications / license : RN licensure with Ohio Board of Nursing, driver’s license with proof of insurance, BLS
