COVID-19 Vaccine Policy
In accordance with LifeLong Medical Care's commitment to provide and maintain a workplace that is free of known hazards, we have adopted a Mandatory COVID-19 Vaccine Policy to safeguard the health of our employees and their families;
our patients and visitors; and the community at large from infectious diseases, that vaccinations may reduce. This policy will comply with all applicable laws and is based on guidance from the Centers for Disease Control and Order of the California State Public Health Officer.

Responsibilities
- Supervise program case managers including recruitment, hiring, onboarding, evaluation and disciplinary action
- Lead regular case conference and team meetings in collaboration with clinical supervisor (LCSW) and community partners
- Coordinate program data collection and provide management reports for program monitoring and reporting to funders
- Develop and maintain relationships with outside agencies to promote program referrals and collaboration
- Develop and maintain community resources and linkages
- Ensure client referral and intake process supports contract and funding goals
- Identify and develop policies, procedures and workflows to ensure contract compliance, create efficiencies, promote quality and achieve program goals
- Coordinate site tours and shadowing opportunities for partners, funders and potential staff in order to promote SHP programs
- Identify training needs and develop and manage staff training opportunities
- Develop and submit reports to funders and participate in grant management activities
- Coordinate and manage purchasing and equipment to support case management teams and SHP administrative office
- Promote diversity, equity, inclusion, and belonging in the program model and in support of SHP participants and staff
- Perform other duties as assigned by supervisor
Qualifications
Job Requirements
- 2-3 years of experience providing community-based support services for people experiencing homelessness or other special needs populations, especially those with mental health and substance use issues
- 1-2 years of experience in program planning and management
- 1-2 years of team supervision and management experience
- Fluency with MS Suite (including MS Teams) and web meeting platforms
- Excellent written, verbal communication, and organizational skills
Job Preferences
- Bachelor’s degree in a related field (Public Health, Social Work)
- Experience with Clarity (HMIS), Epic, and / or Community Health Record
- Experience with Coordinated Entry System (CES)
