St. Louis, MO 63131).Position Details : * Day Shift Hours : Mon-Fri (8am-4 : 30pm or 7 : 30am-4pm) with rotating weekend & holidays* Case Management in an acute medical hospital setting* Assessments, discharge planning, & addressing psychosocial needs* Collaborate with RN CM to deliver medical case management and discharge planning services in an acute hospital setting.

Louis County, offers a full continuum of medical and surgical services, including heart care, cancer, women and infants, breast health, gastrointestinal, orthopedic, stroke, therapy, wound and pain management services.

The hospital has a 24-hour adult emergency department and cares for pediatric patients at a separate emergency department in collaboration with St.

Louis Children's Hospital.Missouri Baptist is the first and only hospital in St. Louis County to be named a Magnet hospital by the American Nurses Credentialing Center (ANCC).

The prestigious designation is the highest credential a health care organization can receive for nursing excellence and quality patient care.

This recognition has been accomplished by less than nine percent of hospitals nationwide. Additionally, U.S. News & World Report ranked the hospital #2 in the St.

Louis metro area.The Care Coordination Department is comprised of RN Case Managers, Social Workers, Compliant Documentation Specialists and skilled administrative staff.

Case Management in the Hospital setting is a collaborative practice model including patients, nurses, social workers, physicians, other practitioners, caregivers and the community.

The Case Management process encompasses communication and facilitates care along the continuum through effective resource coordination.

The goals of Case Management include the achievement of optimal health, access to care and appropriate utilization of resources, balanced with the patients right to self-determination.

Preferred QualificationsRole PurposeUnder direction of a physician, assists patients and their families in their residences or outpatient / inpatient settings to adapt and plan an interdisciplinary approach to treating the patient-family system.

Employees working at an Illinois facility are required to have an Illinois state social work license or be able to take and pass the Illinois state social work licensing exam within 90 days of the first day of employment.

ResponsibilitiesProvides direct clinical services to patients and families.Plans and implements social work services on assigned clinical service or unit in collaboration with the healthcare team.
