Speech Language Pathologist
We are seeking a Speech Language Pathologist for an opening with our Home Health team based in Concord, CA! We believe that home healthcare is more than just a service it’s a commitment to enhancing the quality of life of every individual we serve.

Compensation Package :
- $65 to $85 An Hour (Based on Experience )

Requirements :
- Valid CA Speech Pathologist License
- 2+ years of clinical experience as a speech therapist
- 1+ years of working with the adult population in speech therapy
- Maintain CPR Certification
- Home Health, Hospital or SNF Experienced Considered
- Must have and maintain a valid driver’s license, maintain automobile insurance coverage, and have access to an automobile.
Responsibilities :
- Monday through Friday 8 AM to 5 PM - Flexible Hours
- Seeing 5 to 6 Patients a Day in their Homes
- Coverage area is based on the Speech Therapists home location
- Documentation Kindly Asked to be Completed Within 24 Hours
- No COVID or Flu Vaccines Required
- Conducts appropriate evaluations, assesses home environment (as appropriate), and identifies equipment needs relative to speech / language function.
- Assumes responsibility for the ongoing interdisciplinary assessment and development of the individualized plan of care in partnership with the patient, representative, and caregivers.
- Provides services that are ordered by the physician as indicated in the plan of care.
- Provides patient, caregiver, and family counseling.
- Provides patient and caregiver education.
- Prepares clinical notes.
- Communicate with the physician who is responsible for the home health plan of care and other health care practitioners related to the current home health plan of care.
