Responsibilities :
- Meets expectations of theapplicable OneCHRISTUS Competencies : Leader of Self, Leader ofOthers, or Leader of Leaders.
- Prepares anddispenses drug orders per physician request according toestablished policies, procedures and protocols
- Interprets drug orders (verbal and written)and transcribes / verifies into computerized patient medicationrecord accurately
- Maintains, accurate,complete patient drug record
- Compounds anddispenses pharmaceuticals including non-sterile and sterileproducts, chemotherapy and parenteral nutrition products accurately
- Issues controlled substances to patient careareas and maintains records as required by law and institutionalpolicies
- Provides services efficiently and ina timely fashion
- Maintains competencyrequired for current job title / position
- Maintains current pharmacist license
- Ensuresapplicable CE records and licensure are maintained in departmentfiles
- Attends staff meetings
- Completes all competency / skills assessmentrequirements
- Provides analysis of medicationuse processes and contributes at least one related project per year
- Supervises and directs pharmacy supportpersonnel
- Verifies the daily activities ofpharmacy technicians
- Ambulatory care
- Provides ambulatory pharmaceutical careservices utilizing approved protocols and / or collaborativeagreement with other providers, including managing patient drugtherapy, anticoagulation management, providing patient education,and preparing and / or overseeing the preparation and dispensing ofmedications
- Performs duties in the clinicalareas assigned such as but not limited to : anticoagulation clinics,intensive medical home, transition of care, Discharge Medicationreconciliation, etc
- Monitors drug therapyregimens for contraindications, drug-drug interactions, drug-foodinteractions, allergies and appropriateness of drug and dose
- Assists with pharmacokinetics consult serviceand renal drug dosing per hospital protocol
- Reads, extracts and interprets information in patient medicalrecords accurately
- Detects and reportssuspected adverse drug reactions accurately and in a timely manner
- Sustains the hospital drug formulary,minimizing non-formulary procurements, utilizing therapeuticsubstitution protocols and promoting rational drug therapyselection
- Provides clinical consultation andclarification to practitioners as appropriate
- Provides accurate, adequate and timely drug information to thehospital’s professional staff
- Provides drugeducation to patients and their families per institutional protocol
- Participates in the quality improvement andmedication use review activities of the department
- Collects data, conducts monitors andinspections and maintains logs, records and other documentation asassigned
- Conducts designated interventions asdefined in department Clinical Intervention activities (IV to PO,Renal Dosing, Pharmacokinetic dosing support, Anticoagulation)
- Baseline departmental standards for reportinginterventions for clinical productivity as defined by departmentalgoals
- Participates in antibiotic stewardship,opioid stewardship, and shortage management activities as assigned
- Attends interdisciplinary rounds whenassigned
- Provides services efficiently and ina timely fashion

Education / Skills
Enrolled and in good standing with an ACPEaccredited School of Pharmacy program required
Successful completion of the firstprofessional year and a minimum of 30 credit hours of work towardsa professional degree in pharmacy required
Experience
