HOW YOU WILL MAKE AN IMPACT AS A HOSPICE RN :
- The RN respects and promotes the mission, vision and values of Ahava Hospice
- Provide hospice patient care as needs arise for our patients
- Enable patients to spend quality time with their loved ones and doing the activities they enjoy
- Build trusting compassionate relationships with your patients and their families
- Empower caregivers by educating them on care for their loved one
- Help the patient and family maintain their maximum level of comfort and coping
- Acts as Case Manager and assumes responsibility to coordinate patient care for assigned caseload.
- Completes an initial, comprehensive, and ongoing comprehensive assessment of the patient and family to determine hospice needs.
- Develops an individualized care plan that establishes goals based on nursing diagnosis and incorporates palliative nursing actions.
Includes the patient and family in the planning process.
Communicates with the physician regarding the patients' needs and reports changes in the patient's condition; obtains / receives physician’s orders as required.
Communicates with community health related persons to coordinate the care plan.
- Provides health care instructions to the patient as appropriate per assessment and plan.
- Attends interdisciplinary group meetings
- Participates in on-call duties as defined by the on-call policy
- Supervises ancillary personnel and delegates responsibilities when required.
- Assumes responsibility for personal growth and development and maintains and upgrades professional knowledge and practice skills through attendance and participation in continuing education and Inservice classes.
- Fulfills the obligation of requested and / or accepted case assignments
POSITION QUALIFICATIONS
- Graduate of an accredited school of nursing.
- 1-2 years of recent acute care experience in an institutional setting and 1-2 years recent experience in hospice care preferred.
- Must be a licensed driver with an insured automobile in accordance with state and / or organization requirements in good working order.
- Bachelor's degree with one year of hospice care experience preferred.
- Excellent observation, verbal and written communication skills, problem solving skills, basic math skills, nursing skills per competency checklist.
- Prolonged or considerable walking, standing, sitting may be required. Able to lift, and / or transfer patients, supplies and equipment will be necessary.
- Considerable reaching, stooping, bending, kneeling, and / or crouching. Visual acuity and hearing to perform required nursing skills.
