Immediate opportunity for a Certified Nurse Aide to perform in-person ADL / home health aide visits under a Plan of Care developed by the Registered Nurse Case Manager.
Requires driving personal vehicle to patient homes to provide care. 20-25 visits per week.
Will require any of the following as ordered :
- Shower, tub bath, bed bath or sponge bath as ordered
- Hair Care
- Oral Care and Hygiene (dentures)
- Safe transfers in and out of bed / chair / wheelchair / walker / shower
- Ambulate with assistance (walker / gait belt / stand by assist)
- Turn and position
- Range of Motion Exercises (Home therapy program)
- Take and record Vital Signs
- Perineal Care as ordered
- Prepare meals
- Light Housekeeping
