Home Hospice Social Worker
Associate license (LSWAIC) required with commitment to obtain LMHC, LMFT, LASW, or LICSW within 3 years.
Washington State (DOL) driver's license required. Reliable transportation with good driving record required. Private auto insurance required.
Basic Life Support (BLS) certification required.
ROLE SUMMARY
- The Social Worker is responsible for assessing / re-assessing and providing support and intervention to address the psychosocial / emotional needs of patients and their families, and implementing effective strategies to meet such needs, including related counseling services;
- establishing and maintaining relationships with community health and social work agencies related to client’s needs; collaborating with multidisciplinary team and interagency personnel to provide for an optimum environment of care;
evaluating results achieved to enhance and ensure consistent quality of care.
ESSENTIAL RESPONSIBILITIES
- Competent to care for patients from diverse religious and cultural backgrounds, recognizing various age related needs.
- Conducts relevant mental health screenings and psychosocial assessments to medically referred and other clients.
- Provide short term, structured, evidence based psychotherapy as part of an appropriate treatment plan for patients with DSMV diagnosis.
- Evaluates social needs of patients and families by arranging interviews, making evaluation and follow-up home visits as indicated in the plan of treatment and allowed under reimbursement guidelines and assessing the financial resources of the patient / family when appropriate in relation to medical and health needs.
- Coordinate and collaborate with the primary care providers to improve health outcomes.
- Facilitate referrals to other services (e.g. substance abuse treatment, specialty care and community resources) as needed.
- Advocates on behalf of patients and caregivers for service access, and for the protection of the patient’s health, safety and rights.
In addition, advocates on behalf of the organization to promote safety, quality and appropriate resource utilization.
- Provide clinical therapeutic services to patients, including crisis intervention, individual and group therapy and coordination of treatment for participants with co occurring disorders.
- Have a working knowledge of legal documents such as Guardianships, Durable Power of Attorney, POLST, Living Wills, and Advance Directives, including legal authority for decision making in healthcare absent the applicable Durable Power of Attorney.
- Have a working knowledge of issues & referral sources associated with dementia, care-giving, care-giver stress / burnout, elder & child abuse & domestic violence, grief & loss.
Promotes safety, quality and appropriate resource utilization.
- Ensures compliance with state reporting requirements and appropriate documentation as identified by Hospital policy and regulatory requirements / agencies.
- Provides consultation to staff and physicians regarding reporting requirements.
- Work to meet patient needs effectively and resolve individual barriers through follow up, advocacy and collaboration with other service providers.
- Maintain accurate and timely records of activities, case management notes, and services provided to each client.
- Serves as a liaison between Hospital, family and community resources; serves as a source of information regarding community resources
- Provides consultation to staff and physicians on patient issues and provides protocol clarification / guidance.
- Develops and maintains professional relationships with medical staff, clinical team, community providers and payers to promote effective coordination of care across continuum of services.
- Attends and actively participates in committees as appropriate for service line.
- Accepts responsibility for maintaining and improving professional competence.

Responsibilities
You will administer assessment tools and gather data for assessment, and to assess for optional needed services
You will interview families to determine social work needs of the participant
You will contact community resources to coordinate services
You will provide initial and on-going assessment of participant's social, emotional and cognitive status and needs, involving participant and other staff members in the assessment
You will ensure accurate documentation is maintained for all assessments, treatment plans, and evaluations
Requirements
Master's Degree in Social Work
LSWAIC, LMHC, LMFT, LASW, LICSW as required by department
Experience working in a hospital / acute situation or equivalent social service agency, experience relative to the medical social work function

Pay and Benefit Expectations
We provide a comprehensive benefits package, including competitive salary, medical, dental and retirement benefits and paid time off.
As required by various pay transparency laws, we share a competitive range of compensation for candidates hired into each position.
The pay scale is $34.47 - $48.26 USD. However, pay is influenced by factors specific to applicants, including but not limited to : skill set, level of experience, and certification(s) and / or education.
If this position is associated with a union contract, pay will be reflective of the appropriate step on the pay scale to which the applicant’s years of experience align.
