Licensed Nutrition Professional
JOB SUMMARY :
This position involves assessing the nutritional status of PACE participants, planning and implementing appropriate nutrition interventions, and collaborating with the Interdisciplinary Team to ensure the nutritional needs of participants are fulfilled. You will provide nutrition counseling, education, and therapy to participants while also communicating effectively with their families and caregivers. Additionally, you will participate in developing personalized care plans and assist in the coordination of continuous care delivery.
ESSENTIAL DUTIES AND RESPONSIBILITIES :
Deliver high-quality nutritional care as an integral member of the PACE interdisciplinary team.Conduct initial comprehensive nutritional assessments at the time of enrollment and as needed.Collaborate with the interdisciplinary team to create and update comprehensive care plans for each participant.Provide nutritional counseling to participants, caregivers, and family members to promote health and wellness.Facilitate nutrition-related interventions, such as home-delivered meals and nutritional supplements.Monitor weight, medical conditions, drug-nutrient interactions, and any changes in the participant's health, reporting findings to the interdisciplinary team.Engage in communication and relationship-building with coworkers, participants, families, and community members.Participate in all interdisciplinary team meetings.Assist interdisciplinary team members in understanding the nutritional needs related to participants' health issues.Incorporate cultural and ethnic preferences into care plans.Perform the duties of the Food Service Supervisor as assigned.Actively participate in Quality Improvement projects and meetings.Attend family and staff meetings, training, and orientation programs as required.Follow all PACE Program Policies, Procedures, and Occupational Safety and Health Administration (OSHA) guidelines.Ensure confidentiality and privacy of sensitive information.Document care clearly and comprehensively.Adhere to standard precautions and infection control protocols.Complete position-specific competencies before assuming participant care.Perform additional duties as required or requested.EXPERIENCE AND EDUCATION :
Bachelor's degree in food, nutrition, or dietetics from an accredited college.Passage of the Commission on Dietetic Registration (CDR) exam.Minimum of 1 year working with the elderly population is required; training will be provided if applicable.Preferred : Minimum of 2 years of clinical nutrition experience.Preferred : Serve Safe Certification.Registered or eligible for registration by the Commission on Dietetic Registration.PRE-EMPLOYMENT REQUIREMENTS :
