You will follow up a patient's CANS evaluation to provide information about health home options or alternative community support resources.
The Family Support Coordinators are the second line of support for children and their families after their eligibility determination is done.
This role is phone based, with no face to face contact or home visits.
Salary : $21.63 / hour
Days / Hours : Monday- Friday Day hours, plus occasional Saturday hours
