Department : Behavioral Health Manager Title : Chief Population Health Officer

Job / Grade : DOL Status : Exempt Job Summary : The Behavioral Health Director is responsible for leading and directing PCC's mental health / substance abuse services.
Essential Duties and Responsibilities :
Provide clinical assessment and action plans for patients. Identify appropriate resources and follow-up on all referrals made.
Participate with multidisciplinary team to ensure holistic psychosocial and medical needs are met. Establish a formal relationship with community and social service agencies.
Provide consultation and academic support to residents and attending physicians in the areas of behavioral medicine. One half of the manager time is dedicated to administrative work and one half to clinical practice.
- Maintain 20 patient care hours per week at designated site as determined by agreement with the Chief Medical Officer and following PCC's policies for scheduling, access, chart documentation and availability for communication regarding clinical matters during regular business hours.
- Provide onsite mental health consultation regarding assessment and mental / behavioral health treatment options to primary care physicians and allied health providers.

- Participate in individual and group supervision.
- Perform any crisis intervention, individual counseling, family counseling, advocacy that is needed for the patient.
- Supervision and management of behavioral health personnel.
- Behavioral Health grant management and oversight.
- Westside Healthy Start grant management and oversight of grant funded staff.
- Collaborate with Clinical Support Services Manager for peer review, provider quality incentive program and quality indicators quarterly.
- Recruit and retention of qualified behavioral health center staff.
Marginal Functions :
- Must demonstrate ability to effectively and efficiently handle a demanding workload involving multiple tasks.
- Basic computer skills in MS Word, in electronic medical records (EMR), accessing emails and doing internet research.
- Ability to communicate effectively in oral and written communications.
Experience / Training :
Demonstrated skills in the clinical social work practice arena and the ability to work and collaborate in a health care team.
Certifications / Licenses :
- Clinical Social Worker License (LCSW) / LCPC / LPC
- Master's Degree in Social Work.
- Knowledge and experience in health administration and policy preferred.
- Bilingual English / Spanish is preferred.
Physical Demands :
- Ability to stand, bend and stretch.
- Ability to travel and maneuver between site locations if needed.
- Ability to lift 10 pounds.
- Ability to type for long periods of time.
Language Skills :
- Ability to read and write proficiently using the English language.
