- Location : Lancaster, PA
- Company : Sunbelt Staffing, on behalf of a Nationwide Home Health Agency
- Position Type : Contract, 13-week term with possibility of extension upon successful completion
- Schedule : Monday-Friday, 8 : 00 AM - 5 : 00 PM with occasional weekends and on-call
- Compensation : Competitive salary, mileage reimbursement, medical, dental, vision, and 401k benefits after 90 days
- Sick pay after 6 months of employment
Job Responsibilities :
- Conduct home health visits to patients homes within territory and surrounding area.
- Perform comprehensive assessments of patients' health status, including physical, emotional, and environmental factors influencing their well-being.
- Develop and execute personalized care plans tailored to meet the unique needs and goals of each patient, in collaboration with the healthcare team and under physician guidance.
- Administer skilled nursing care, encompassing wound management, medication administration, IV therapy, and patient education on disease management and preventative care measures.
- Maintain accurate and timely documentation of all patient interactions, assessments, interventions, and outcomes utilizing Oasis and Homecare Homebase software platforms.
- Foster interdisciplinary collaboration by actively participating in team meetings, sharing insights, and coordinating care transitions to ensure continuity and effectiveness of care delivery.
- Establish and maintain open lines of communication with patients, families, and caregivers, providing support, education, and advocacy to enhance patient understanding and engagement in their healthcare journey.
Job Requirements :
- Active PennsylvaniaRegistered Nurse (RN) License.
- Active American Heart Association (AHA) or American Red Cross Basic Life Support (BLS) certification.
- Minimum of 1 year of experience in home health nursing.
- Minimum of 1 year of experience with Oasis and Homecare Homebase software.
- Excellent clinical assessment and critical thinking skills.
- Strong communication and interpersonal abilities.
- Reliable transportation and willingness to travel within assigned territory.
