JOB SUMMARY
The Lead RN Case Manager supervises a team of RN Case Managers in the inpatient and emergency departments at all NCH campuses.
This role involves leadership, team-building, communication, and problem-solving. The Lead RN Case Manager also plays a key role in program development, staff training, and community outreach.
ESSENTIAL DUTIES AND RESPONSIBILITIES
- Support the Director of Case Management and Social Work Manager in implementing and enforcing NCH and departmental policies and procedures.
- Supervise, orient, and mentor RN Case Managers.
- Reinforce compliance with department documentation standards of initial comprehensive assessments, readmission assessments, progress, and discharge notes.
- Ensure comprehensive individualized discharge planning that meets the needs of patients and their families.
- Identify gaps in services and develop strategies to improve patient care and resource utilization.
- Participate and promote quality of care initiatives, length of stay, readmissions, and avoidable days.
- Provide patients / families with resources to meet their medically related social and emotional needs.
- Ensure safe transition of patients from one care environment to another.
- Promote patient / family involvement and ensure choice in discharge planning.
- Collaborate with multi-disciplinary teams, healthcare providers, and community partners to ensure comprehensive patient care and care transition.
EDUCATION, EXPERIENCE AND QUALIFICATIONS
- Minimum of Associate Degree in Nursing required; BSN preferred.
- Minimum 2 years of supervisory / manager-level experience in hospital Case Management required.
- Licensed as a Registered Nurse in the state of Florida.
- Basic Life Support (BLS) certification required from the American Red Cross or American Heart Association.
- Case Management Certification preferred.
- Intermediate computer knowledge : Uses Microsoft Word, Excel, Outlook, and Windows.
