POSITION SUMMARY : Responsible for providing initial evaluations, individual, group, couples and / or family therapy at the clinic.
Therapists will work collaboratively with the primary care providers on treatment plans in the ambulatory setting. Act as advocates and help clients gain access to resources while confronting their personal issues, such as mental illness, addiction, and abuse.
Assist the care coordination team (provider, nurse, care coordinators, etc.) by providing behavioral health services to patients and their families in the ambulatory setting to enhance psychosocial functioning and continuity of care.
Provides advocacy, information and referral services to patients and families to address their medical and psychosocial needs.
Provides assistance with overall program development, community development, training and education in area of responsibility.
Participates in continuous quality improvement activities and educational experiences in support of departmental philosophy and objectives, as well as, Health System initiatives.
EDUCATION / TRAINING : Master#s degree required. LCSW license or LCPC. At least two years of experience providing clinical services in mental health.
At least two years experience in individual and / or group casework preferred. LICENSURE / CERTIFICATION : Current license from the State of Illinois.
Valid State driver#s license and proof of liability insurance.# Bi-Annual CPR certification. EXPERIENCE / SKILLS / ABILITIES : Successful completion of annual clinical job specific competencies and skill verification tools required.
Knowledge of principles of growth and development over the life span. Good interpersonal skills and ability to work with patients, physicians, families, managers and members of the community under varied conditions.
Excellent organizational, critical-thinking and conflict resolution skills required.# Tolerance of stress and the ability to recognize your own limitations and respond to difficult situations.
The ability to apply your knowledge and research to clinical problems. The capacity to be critical and analytical. To work in a self-motivated, independent way.
