Certified Diabetes Educator or Certified Diabetes Care and Education Specialist
Day Shift, 8a 5p
Location : Bedford, NH
- Reports to the Department Manager.
- Primarily responsible for outpatient diabetes education in collaboration with multidisciplinary team and the referring provider.
- May also provides medical nutrition therapy services to patients and / or families including assessment of nutritional status, nutrition care planning, counseling and evaluation.
- Interacts with other health care team members in information exchange for planning and delivering appropriate patient care.
- Provides diabetes education to patient and staff.
- Practices under the guideline of the Standards of Practice and Standards of Professional Performance for Registered Dietitians in Diabetes Care’ published by the Academy of Nutrition and Dietetics.
Qualifications :
- Minimum of a Bachelor of Science in Nutrition.
- 2+ years of experience in clinical dietetics and as a diabetes educator. Past experience with Bariatric population preferred.
- Certification / Licensure : Registered Dietitian. Current certification as a Diabetes Educator by NCBDE (National Certification Board for Diabetes Educator) or certification as a Certified Diabetes Care and Education Specialist (CDCES) by CBDCE (Certification Board for Diabetes Care and Education) required.
Certified pump trainer preferred.
Proficient in computer use. Some experience in Electronic Medical Records system is preferred.
