The Director of Case Management isresponsible and accountable for the implementation of the casemanagement program at the local level.
The components / roles of theinpatient case management program consist of the following : CareFacilitation Utilization Management Case Management and DischargePlanning.
PrincipalAccountabilities
- Plans directs and supervises all aspects of the locallevel program.
- Facilitates growth anddevelopment of the case management program consistent withenterprise wide philosophy and in response to the dynamic nature ofthe health care environment through benchmarking for best practicesnetworking quality management and other activities asneeded.
- Responsible for approving and managingthe day to day local level operational budget.
- Assures that revenue expenses contribution margin andFTE’s meet or exceed budget.
- Prepares and submits budget and relatedreports.
- Forecasts and accurately projectsexpenses.
- Takes corrective action to addressnegative variances.
- Identifies and proposescapital budget items appropriately.
- Identifiesand achieves optimal targeted financial outcomes via the inpatientcase management process.
- Responsible fordepartmental personnel functions (hiring firing etc.) inconjunction with the Executive Director of MedicalManagement.
- Writes and conducts annual andinterim performance appraisal reviews for the professional andnonprofessional staff in department.
- Acts asliaison to facilitate communication and collaboration between allcare partners (physicians hospitalists community care managersnurses community resources etc.)
- Responsiblefor leading a high performance team of systemthinkers who incorporate leadership principles andvision in performing the functions of case management.
- Uses data to drive decisions plan and implementperformance improvement strategies for case management.
- Oversees the education of physicians managers staffpatients and families related to the case management process at thelocal level.
- Participates in thisevolutionary process by constantly identify future needs of currentcustomers and / or identifying potential new customers.
- Ensures safe care to patients staff and visitors; adheresto all Memorial Hermann policies procedures and standards withinbudgetary specifications including time management supplymanagement productivity and quality of service.
- Promotes individual professional growth and developmentby meeting requirements for mandatory / continuing education andskills competency;
supports departmentbased goals which contributeto the success of the organization; serves as preceptor mentor andresource to less experienced staff.

Requirements
Licenses / Certifications :
- Current and valid license to practice as a RegisteredNurse in the state of Texas or
- Current andvalid Texas license as a Master’s Social Worker (LMSW)required
- LCSW preferred and Certified CaseManager (CCM) Accredited Case Manager (ACM) or
- Fellowship of the American Academy of Case Management(FAACM) required
Education : Bachelors of Nursing (BSN) or MastersSocial Work (MSW); Masters degree preferred
