About The Position
Weekly Pay Range - $2126 To $2362 Job Title : Physical Therapist (PT) Home Health Job Specialty : Therapy / Rehabilitation Job Duration : 13 weeks Shift : Day 5x8-Hour (08 : 00 - 17 : 00) Guaranteed Hours : Not guaranteed Experience : Minimum 1 full year of Adult Home Health Experience or 1 full year of Hospice Experience License : Must have an active license in hand Certifications : CPR, BLS Must-Have : - Proficiency in OASIS for RNs, PTs, and OTs- Experience with HomeCare HomeBase (HCHB) within the last year- Reliable transportation, a current driver's license, and agency-required liability insurance Job Description : Provide, coordinate, and direct the provision of home nursing care according to physicians' orders and agency policies and procedures.
- Directly supervise and evaluate Home Health Aides and Licensed Practical Nurses / Licensed Vocational Nurses.- Conduct home visits with a typical daily patient load of 5-10, equating to roughly 6-8 points.
- Engage in different types of cases, including Start of Care, Wound Care, Follow-up Visits, etc.- Attend periodic office meetings and demonstrate traveling flexibility, willing to travel up to 75 miles from the home clinic.
- Participate in one weekend rotation per month and cover care center territory locations as needed.- Proficiency with HomeCare HomeBase EMR systems is required.
- Maintain an average of 30 productivity units per week.- Support growing census needs due to PTs on leave.*Note : COVID-19 vaccine is not required, and flu vaccine requirements are unknown.*
