About the Department

The Pediatric and Adolescent HIV / AIDS Program, Care2Prevent, in the section of Pediatric Infectious Diseases provides comprehensive treatment and prevention services for people living with HIV and those who are most vulnerable to HIV infection.

Job Summary

The Medical Case Coordinator advocates for patients / clients, identifies obstacles to maintaining ongoing engagement in HIV care, and maintains appropriate documentation of client interactions and services provided.

The Medical Case Coordinator supports outpatient medical clinics to ensure eligible patients receive case management services and helps to monitor the weekly clinic schedule to ensure appointment adherence.

The Medical Case Coordinator attends interdisciplinary team meetings, mandatory trainings, case conferences, participates in pager coverage, and community outreach / education.

Upon completion of training, the Medical Case Coordinator is expected to obtain certification in HIV case management from the AIDS Foundation of Chicago within 90 days of employment.

This position is wholly funded by contractual grant funding which is renewed under provisions set by the grantor of the contract.

Employment will be contingent upon the continued receipt of these grant funds and satisfactory job performance.

Responsibilities

Provides one-on-one support, referrals to psychosocial support services, addresses health insurance coverage needs, assists with and / or arranges transportation, food and housing, and other resources as needed.

Maintains client privacy and confidentially in compliance with HIPAA guidelines as well as policies outlined by the Care2Prevent program, the University, and funding agencies.

Maintains detailed client files including case notes, referral requests, and other reporting documentation as required by the Care2Prevent program, the university, and funding agencies.

Maintains regular contact with clients to support adherence to care and performs home visits as appropriate (some local travel).

Occasional evening hours are required.

Screens and assesses individuals and / or families to determine education and counseling needs. Counsels individuals and / or on the basic principles of health education.

Supports individual and community health by contributing to the building and maintaining working relationships with those in public health care.

Serves as an educational resource and patient advocate.

Performs other related work as needed.

Minimum Qualifications

Education :

Minimum requirements include a college or university degree in related field.

Work Experience :

Minimum requirements include knowledge and skills developed through < 2 years of work experience in a related job discipline.

Certifications :

Preferred Qualifications

Education :

Bachelor's degree in Social Work or related human service, social science, or health education field from an accredited college or university.

Experience :

1-2 years of related social service experience.

1-2 years demonstrated experience working with complex pediatric and adolescent individuals.

Lived experience in communities of focus is a plus.

Preferred Competencies

Sensitivity to and understanding of issues concerning HIV.

Experience working with diverse populations including that of minorities and LGBTQIA+.

Familiarity with group facilitation and / or willingness to learn.

Detail-oriented with the ability to act as a liaison between clients and service providers.

Microsoft Office experience and general computer experience.

Ability to adjust well under high stress and ever changing environments.

Attention to detail and ability to prioritize independently.

Ability to maintain confidentiality, problem-solve, reason and communicate effectively in both verbal and written form.

Comfortable with frequent communication via phone and e-mail.

High level of proficiency with time management.
