SSTAR is a Federally Qualified Health Center (FQHC) which affords our employees eligibility to apply for one of our three Loan Repayment Programs.
The Case Manager is responsible for clinical and medical case management for all Lifeline clients.

Duties & Responsibilities
- One on one case management for person with substance abuse disorders.
- Active participation as a member of the multi-disciplinary team that develops and supports the client's service plan.
- Coordinate care / services with SSTAR open access model and primary care needs.
- Awareness of community resources and referral mechanisms.
- Identifies gaps in service and advocates appropriately.
- Maintain confidentiality to the fullest extent.
- Assist with transportation arrangements.
- Represent program / agency internally and externally as requested.
- Accepts and participates in appropriate supervision.
- Other duties as required.
Education & Experience
- Bachelor's degree in social work or related field or CADC.
- Strong verbal and written communication skills.
- Open to working with people from diverse backgrounds, cultures, and lifestyles in a non-judgmental manner.
