LOCATION : 2406 Atherholt Road, Lynchburg, VA. Forest Health and Rehab Center

Established in 1995, Forest Health and Rehab is a 97 bed skilled nursing and rehab community located in Lynchburg, VA, which many may know as "Hill City" or "The City of Seven Hills".

Our facility sits on a cul-de-sac shared with Johnson's Health Center and The Surgery Center of Lynchburg, and is conveniently located near Lynchburg General Hospital.

At Forest Health and Rehab, we provide compassionate specialty services to all residents.

Patient Population : Adult and Geriatric

EMR : Crown Web

Dress Code : Dialyze Direct will provide black scrub tops. Scrub bottoms : Personal scrubs of any color

Requested time off during the course of this assignment MUST be noted and verified at upload. RTO REQUESTS AFTER UPLOAD WILL NOT BE APPROVED!! NO MORE THAN 5 DAYS OF RTO WILL BE ACCEPTED (less is better).

Non-billable orientation : 8hrs

Holiday and OT Multipliers : 1.25%

Requirement description : TRAVELER MUST BE WILLING TO FLOAT TO ADDITIONAL FACILTIIES IF NEEDED

• 1 year or more CURRENT Dialysis experience - Required
• Copy of Current, Valid Driver License - Required
• Geriatric patient experience - Required
• NXStage Dialysis Machine experience - PREFERRED, Please note experience on submission
• CNN or CDN Certification - PREFERRED
• Crown Web EMR experience - Preferred
• BSN - Preferred

• Work History
• Skills Checklist (pertinent to position), current to 1 year
• 1 Supervisory Signed Reference (from within the past two years of employment)
• License / Certifications - Must be uploaded with profile

CERTIFICATION REQUIREMENTS :
