About The Position
Weekly Pay Range - $2810 To $3122 Job Title : Physical Therapist (PT) Job Specialty : Physical Therapist (PT) Job Duration : 13 weeks Shift : 08 : 00 AM-04 : 30 PM Guaranteed Hours : Experience : Minimum 1 year of home care experience required License : Physical Therapist Professional License in Massachusetts Certifications : Must-Have : Ability to travel to assigned cases, weekend rotations every 6 weeks Job Description : Provide physical therapy services to patients in their residences who have physical impairments, functional limitations, or disabilities resulting from injury, disease, or other causes.
- Collaborate with the patient, family, physician, and other allied health professionals to identify patient needs and coordinate services to meet those needs.
- Utilize thorough and timely electronic documentation and interdisciplinary communications in accordance with agency policies, procedures, and standards of practice.
- Work with a diverse patient population, ranging in age from birth to over 100 years.
