Job Description for CNA’s and HHA’s :
- Deliver exceptional care to our patients in the comfort of their home
- Perform comprehensive personalized weekly schedules and care plans
- Complete daily tasks in a safe and professional manner (ADLs)
- Provide personal companionship, actively engage with each patient by providing ongoing social and emotional support
- Assist with personal care activities to include personal hygiene, incontinence care, light housekeeping, mobility assistance, feeding and meal preparation
- Always prioritize safety and patient comfort
- Provide transportation to and from appointments as needed
- Document completion of tasks and report changes in the client’s physical condition or behavior
Job Description for CMT’s :
- Must be certified as a Medication Technician in the state of Maryland
- Must have current CPR Certification
- Preparing and administering prescribed medications to patients under the supervision of a licensed RN, and in compliance with all Agency policies and procedures and Pharmacy Board rules and regulations governing the administration and documentation of medication.
- Adhering to all Agency policies and procedures and Pharmacy Board rules and regulation regarding the administration and documentation of narcotic medications.
- Performing medication administration responsibilities while adhering to all safety principles.
- Adhering to proper infection control techniques during the medication administration process, including the incorporation of universal precautions when indicated.
- Recognizing and adhering to the six (6) rights of medication administration.
- Participating in data collection, medication audits, and other performance improvement activities related to medication administration.
- Reporting any adverse patient medication reactions immediately to the nurse supervisor.
- Ensuring the confidentiality and rights of patients, the confidentiality of the health systems and department documents, and adhering to HIPAA Regulations.
Job Requirements :
- Alzheimer's or Dementia experience is a plus
- Empathy, compassion, and kindness a MUST
- Ability to use sound judgement and remain calm in various situations
- Must be reliable, dependable and on time
- Basic computer skills as well as overall technical savviness
- Proof of eligibility to work in the U.S.
- Ability to pass a background check and provide references
- Proof of a TB test taken within the last year showing clear results
- Certifications for CNA, CMT and HHA
Experience :
One (1) year related experience in a medical environment preferred
Required Credentials :
- First Aid
- Driver's License
- Tuberculosis Clearance
- Authorization to work in the US or Work Permit
- Covid Vaccination Card
