About The Position
Job Title : RN Case Manager Job Specialty : Case Management Job Duration : 8-12 weeks, Full time (40 hours per week). Open to part-time as well.
Shift : 08 : 00 AM - 04 : 30 PM, Days (Monday - Sunday), Holiday rotation required, Weekends required Guaranteed Hours : Not specified Experience : Inpatient Acute Care setting License : Registered Nurse (RN) Certifications : Basic Life Support (BLS) Must-Have : Experience in case management and inpatient acute care Job Description : Provide case management services to inpatient acute care patients.
- Coordinate with medical staff, patients, and families to create effective care plans.- Ensure efficient utilization of resources and facilitate timely patient discharge.
- Create and maintain accurate documentation of case management activities.- Participate in interdisciplinary team meetings to review and update patient care plans.
- Comply with all healthcare regulations and standards.- Engage in continuous professional development activities.- Participate in holiday rotations and work as required on weekends.
