Job Description :
The Admissions Coordinator works under the supervision of the Manager of Case Management to provide coordination for all admissions of every age group and population served at FMC.
Works with referring hospitals, physician offices and clinics to facilitate the admission process.
- Receives initial request for admission.
- Performs utilization review to determine severity of illness and intensity of service, assigns appropriate patient status, and documents in Case Management software according to established protocol.
- Facilitates communication between referring physician and attending physician using 3 way calling.
- Coordinates bed assignment with Bed Coordinator / House Supervisor / Registration.
- Provides timely feedback to referral sources and internal personnel facilitating patient admission.
- Collaborates with Physician Advisor to remove barriers to patient access and / or to facilitate patient throughput within the Center.
- Completes statistical log of referral interaction on an ongoing basis.
Qualifications
Job Qualifications :
- Graduate of accredited school of nursing required, BSN preferred.
- Current Ohio RN License Required, Specialty Certification Preferred.
- Three to five years of recent acute care experience required, critical care preferred.
- Prior experience in case management, pre-admission review, or triage preferred.
Exerting up to 10 pounds of force occasionally (Occasionally : activity or condition exists up to 1 / 3 of the time) and / or a negligible amount of force frequently (Frequently : activity or condition exists from 1 / 3 to 2 / 3 of the time) to lift, carry, push, pull or otherwise move objects, including the human body.
