About the Position :
We are seeking a LPN - Licensed Practical Nurse to work per diem to fill in open shifts. This is an excellent opportunity for those looking to maintain a great work / life balance.

Compensation :
- Hourly Wage Range : $32.00 to $37.40 per hour, depending on years of experience.
- Weekday 2nd and 3rd shift : +$2.00
- Weekend 1st shift : +$2.00
- Weekend 2nd shift : +$3.00
- Weekend 3rd shift : +$3.50
- Per Diem Requirements;
Minimum Shifts : Work a minimum of 4 shifts a month, with at least 2 shifts on weekends (Saturday and / or Sunday).
Job Responsibilities
- Comprehensive Care : Provide care to residents in accordance with CCNH policies, standards of care, and state and federal regulations.
- Care Coordination : Assess, plan, implement, and monitor resident care plans, collaborating with supervisors and attending physicians as needed.
- Supervision : Oversee and support LNAs and LMNAs in delivering quality care on the assigned unit.

Requirements :
- Graduate of an accredited school of nursing
- Long-term care experience preferred
- Must possess and maintain a current Practical Nursing license and be in good standing with the State of New Hampshire Nursing Board
