Job Summary :
The Manager for Case Management services reports to the Case Management Director and is responsible for promoting patient-centered care by coordinating all aspects of hospital-based case management for his / her assigned area of responsibility in alignment with the goals of the Case Management Department.
Job Qualifications
2 year / Associates Degree in Nursing 4 year / Bachelors Degree in Nursing (2-year grace period) OR Masters Degree in Social Work
3+ years experience in Acute-Care Case Management or Social Work
2+ years supervisory experience
