About The Position
Job Title : Physical Therapist Job Specialty : Home Health Job Duration : 13 weeks Shift : Monday-Friday, 0800-1700 Guaranteed Hours : 40 hours per week Experience : Minimum of one year as a Physical Therapist in a hospital, skilled nursing facility, outpatient, or rehabilitation setting.
Home Health experience preferred. License : Current Oregon Physical Therapy License Certifications : Bachelor of Science, Master's, or Doctorate in Physical Therapy from an accredited college or university by the Commission on Accreditation in Physical Therapy Education (CAPTE).
Current CPR / BLS certification for Healthcare Providers. Must-Have : Valid Oregon driver’s license and ability to meet driving requirements Job Description : Provide comprehensive physical therapy evaluation and plan of care for home health patients.
- Administer physical therapy treatment and supervise assistant-provided treatments.- Conduct physical therapy reassessments based on patient needs or regulatory timelines.
- Evaluate patient conditions including orthopedic, neurological, and wound care needs.- Perform detailed assessments including medical history, medication, and environmental safety.
- Manage patient care with an interdisciplinary team for optimum clinical outcomes.- Ensure documentation accuracy, including Oasis reports and clinical evaluations.
- Provide patient, family, and caregiver education for managing health and safety.- Identify and communicate suitable community resources for patient needs.
- Supervise Physical Therapy Assistants and Home Health Aides in delegated tasks.- Manage workload efficiently and maintain productivity standards.
- Participate in orientation and training programs for electronic medical records and departmental processes.
